STEP ONE – Consult and Workup

• Next Step Consult & IVF Team Review

STEP TWO – IVF Treatment

• IVF consult with Dr. Feinberg, Dr. Kovalevsky or Dr. Neithardt
• All nursing consults & calls
• Medication instructions
• All monitoring for IVF & office visits
• IVF clinical procedures: egg retrieval
• IVF lab procedures: fertilization (including ICSI), embryo culture, embryo assessment & assisted hatching, and blastocyst cryopreservation
• Physician consult to discuss outcomes and how/ when FET will proceed
• Embryo storage for 3 months following cryopreservation

Excludes medication, anesthesiologist fees & optional blastocyst biopsy for future chromosome testing prior to cryopreservation.
